Accepting These Terms

These Terms of Service explain how information about you is collected, used and disclosed by the University of Arizona (UArizona,” “we” or “us”) through the Wildcat WellCheck initiative (“Wildcat WellCheck”) when you access or use Wildcat WellCheck SMS, Wildcat WellCheck Self Service website (for enrollment, administration of notification preferences, access submission response instructions, or to access other UArizona provided COVID-19 related information) (collectively, the “Services”), or when you otherwise interact with us.

These terms set out your rights and responsibilities when you use Wildcat WellCheck. By using Wildcat WellCheck you are agreeing to the Terms. If you don’t agree with the Terms, do not enroll nor use Wildcat WellCheck. 

Wildcat WellCheck Purpose

In response to COVID-19 and in support of University of Arizona workplace health, Wildcat WellCheck is used to survey the point in time health of persons prior to visiting UArizona campuses and facilities, and prior to participating in scheduled classes and other UArizona events. As such, Wildcat WellCheck serves as the primary wellness and screening system for employees, DCCs and students of UArizona.


Wildcat WellCheck is available to employees, designated campus colleagues (DCC’s) and students of the University of Arizona. Participants must use their UArizona NetID to complete enrollment and participate in daily wellness screenings including prior to all in person facility visitation, and participation in UArizona classes and events.

Wildcat WellCheck Utilization

When using UArizona’s Wildcat WellCheck, you agree to provide true, accurate, current, and complete information. If your information changes, you agree to promptly update your enrollment to keep it accurate, current, and complete.

During enrollment, you may select which days of the week you regularly report to a UArizona facility and a preferred time and method for Wildcat WellCheck notifications to be delivered to you. According to your notification preferences, you will receive a message reminding you to complete the wellness screening, as well as a confirmation that your screening was successfully submitted. 

Some employees or students may be required to submit more than one wellness screening in a given day, depending on the department in which they work and/or the duties they perform. 

The University of Arizona’s Wildcat WellCheck initiative and daily operation is based at the University of Arizona in the State of Arizona in the United States. By accessing or using this service or otherwise providing information to us, you consent to the processing and transfer of information within and to the United States.

UArizona Privacy and Security Information

We may use the information you provide about yourself for limited and specific purposes. The Wildcat WellCheck initiative subscribes to the University of Arizona’s privacy and security policies and standards, including standards related to cookies and Google Analytics reporting. Smart Phone Text Messaging

Enrolling in Wildcat WellCheck includes provision of a smart phone number for the purpose of receiving SMS notifications directly related to Wildcat WellCheck or to provide COVID-19 related information you have requested. UArizona will not share or use the phone number you have provided for other purposes.

You can cancel the SMS service at any time. Just text "STOP" to the short code. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed from SMS notification delivery. After this, you will no longer receive SMS messages from us. If you want to receive text messages again, just update your notification preferences in Wildcat WellCheck web site, as you did the first time and we will start sending SMS messages to you again.

Carriers are not liable for delayed or undelivered messages and UArizona is not liable for delayed or undelivered messages.

Fees and Costs

The Wildcat WellCheck app and related web services are provided free of charge. However, as is expected when using a mobile device for communications, message and data rates may apply for any messages sent to you from us and sent by you to us. 

Message frequency varies based on your enrollment settings. At minimum, you should expect to receive two messages for each day you will be reporting to UArizona and to send one message to UArizona. When you opt-in to receive additional informational messaging related to COVID 19 and Wildcat WellCheck, you may receive 2-3 additional messages per week.

If you are experiencing issues receiving Wildcat WellCheck text messages you can request assistance by texting the keyword HELP to the short code for assistance, or you can get help directly at WellCheck@list.arizona.edu.


If you subscribe to the Service, there is no guarantee of continuation of the Service, and you will not be entitled to any claim as a result of any service outages that are caused by maintenance on the servers or the technology that supports the Site, failures of our service providers (including telecommunications, hosting, and power providers), computer viruses, natural disasters, or other destruction or damage of our facilities, acts of nature, war, civil disturbance, or any other cause beyond our reasonable control, or discontinuation of the Service at our sole discretion for any reason.

UArizona does not warrant that the Service will be uninterrupted or error free. In addition, no warranty or representation as to the effectiveness of the Service is made. The Service is distributed on an “as is, as available” basis. 

Intellectual Property

This Agreement does not transfer from UArizona to you any UArizona or third party intellectual property, and all right, title and interest in and to such property will remain (as between the parties) solely with UArizona.


These Terms shall be construed in accordance with the laws of the State of Arizona, without reference to its principles regarding conflicts of laws, and the laws of the United States.

Changes to the Terms of Service

We may change the Terms of Service from time to time. If we make changes, we will notify you by revising the date at the top of the policy and, in some cases, we may provide you with additional notice such as sending you a notification through our Services. We encourage you to review the Terms and Conditions whenever you interact with us to stay informed about our information practices and the ways you can help protect your privacy.
